Child/Adult SkilledNursing
At Triumph PHC we know that loved ones may require in-depth medical care due to age or acute medical condition. We provide skilled and unskilled nursing care to seniors, adults and children at-home by highly trained Registered Nurses and Licensed Practical Nurses. We offer compassionate nursing care, comprehensive clinical observation and professional assessment with careful RN supervision. Nursing services include: -Ambulation -Transfer -Positioning of clients -Observation -Assessments -Wound care -Catheter care -Ventilator Care -IV therapy -Diabetic care/teaching -Lab blood draws -Injections -Colostomy care and teaching -Tracheostomy care -Enteral/parenteral feeding care/teaching -Parenteral feeding -Post op/ recovery care -Medication Administration/ Teaching -Medication Monitoring
Child/Adult Personal Care Assistance
Our personal care services are provided to individuals who require assistance with activities of daily living. In addition to caring for elderly adults and children, our personal care providers also work with disabled individuals and those recovering from illness, injury, or a hospital stay. Through personal care, day-to-day life can be made safer, more comfortable, and more livable for these individuals. Personal care services include: -Bathing -Toileting -Grooming -Shaving -Dental care -Dressing -Eating -Ambulation -Transfer of clients -Medication Assistance -Taking of vital signs -Basic First Aid -CPR -Home management -Home safety and sanitation -Infection control in the home -Proper Nutrition
Companion/Sitter
If your family member, friend, or loved one enjoys their independence but is struggling with everyday activities, companion care is the solution. At Triumph PHC our companion care services provide the social interaction and helping hand many need to live happily and comfortably at home.
